New Patient Registration
Please collect the relevant registration forms from reception between 8.30am and 6.00pm
If you intend to live in the practice area, as outlined on the map above, for more than 6 months you may apply to register as an NHS patient. If you are from outside the EU and you have a visa which states you are eligible for treatment on the NHS, please bring your visa along with you to reception when you register.
Patients requesting to join the practice list will not be discriminated against on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition If you are in the area for less than six months you may be able to receive treatment on a temporary basis.
